IMPORTANT PUBLIC ANNOUNCEMENT

Keeping the convenience of aspiring candidates and their parents, the Admission Kits for admission in First Year MBBS/BDS in Government Medical / Dental Colleges of the Punjab, Session 2011-12, shall also be available at Rawalpindi Medical College (RMC) Rawalpindi and Nishtar Medical College (NMC) Multan. Eligible candidates can now receive Admission Kits and submit the duly filled admission forms at UHS, RMC and NMC from 5th October to 15th October, 2011, (Excluding Sunday) from 09:00 AM to 03:00 PM. Candidates are advised to attach all required documents with their admission forms. Admission forms, incomplete in any respect, shall be rejected. The rest of the conditions shall remain the same as laid down in the advertisements of 22.09.2011 and 01.10.2011 respectively.

IMPORTANT PUBLIC ANNOUNCEMENT

This is in partial modification of the advertisement appeared on 22.09.2011.

  • Keeping in view the convenience of aspiring candidates and their parents / relatives, especially those residing in the far flung areas of the Punjab, all eligible candidates are now allowed to receive the Admission Kits and submit their duly filled Admission Forms at University of Health Sciences (UHS) Lahore from 5th October to 15 October, 2011, (excluding Sunday) from 09:00 AM to 03:00 PM, through a One-Window Operation.

  • The candidates are advised to bring following documents with them to be attached with the form:

    1. Three (03) attested copies of Secondary School (Matriculation) Certificate. Detailed Marks Certificate issued by the Board is also acceptable. Candidates having foreign qualifications must provide the attested copies of Equivalence Certificate issued by IBCC.

    2. Three (03) attested copies of Birth Certificate issued by Union Council, if date of birth is not mentioned on Secondary School (Matriculation) Certificate.

    3. Three (03) attested copies of Higher Secondary School (FSc Pre-Medical) Certificate issued by the Board. Candidates having foreign qualifications must provide the attested copies of Equivalence Certificate issued by IBCC.

    4. Three (03) attested copies of Entrance Test Result Card issued by UHS.

    5. Three (03) attested copies of Domicile Certificate of the candidate. No other certificate or document (e.g. Birth Certificate, B-Form, CNIC etc.) is acceptable in lieu of Domicile Certificate of the candidate. Any candidate found to have domicile of more than one place shall be disqualified.

    6. Three (03) attested copies of Character Certificate from the institution last attended.

    7. Three (03) attested copies of CNIC of Father / Guardian.

    8. Four (04) recent passport size photographs. One to be pasted on the Admission Form and to be attested on the front, and other three, attested on the back, to be attached with the form.

    9. Original Fitness Certificate issued by a registered medical practitioner / Government Medical Officer.

    10. Original Affidavit on Rs.20/- stamp paper as per specimen given as Annexure-I on UHS website www.uhs.edu.pk.

    11. Candidates claiming to be Muslim Huffaz-e-Quran shall attach three (03) attested copies of Hifiz-e-Quran Certificate issued by a registered Madrassa.

    12. Candidates claiming to be disabled students shall attach Original Disability Certificate as per specimen given as Annexure-III on UHS website www.uhs.edu.pk , issued by a certified specialist working in a Government Hospital.

    13. Candidates seeking admission against seats reserved for Under-Developed Districts (D.G. Khan, Muzzafargarh, Rajanpur, Lodhran, Bhakkar, and Layyah) shall attach Original Certificates from the Heads / Principals of the concerned educational institutions where they have studied from class one to twelve. These certificates must be dully verified by concerned EDO (Education) and DEO (Colleges) and countersigned by the DCO of relevant district. These candidates will also submit a Surety Bond on Rs.20/- stamp paper as per specimen given as Annexure-IV on UHS website www.uhs.edu.pk.

    14. Candidates already admitted in any medical and dental institution and seeking fresh admission shall have to disclose their previous admission by submitting a certificate as per specimen given as Annexure-V on UHS website.

  • The documents attached with the Admission Form must be attested by a Gazzeted Government Officer in BS 17 or above. The stamp of the officer must bear his / her full name, designation and current place of duty.

  • The candidates are advised to also bring their original certificates / testimonials with them.

  • The candidates are required to declare their choices for medical / dental institutions, in order of preference, in the space provided for that purpose in the Admission Form. Candidates are strongly advised to think carefully before declaring their preferences. Preference once given shall be final and cannot be changed subsequently. They must write down full names of the institutions, as per their choice. Do not use abbreviations. Cutting or erasing is not allowed. This condition is mandatory and neither any subsequent change is entertainable nor any exceptions will be made. The candidates shall be considered only for those colleges for which they have shown their preference. A candidate, under no circumstances, will be considered for a college not listed in his/her preferences.

Applications are invited on prescribed Admission Forms, from candidates having Domicile (No other certificate is acceptable) of any district of Punjab or Federal Capital Area (Islamabad) and who have appeared in the Entrance Test held on 13th September, 2011, for admission to Government Medical and Dental Colleges of the Punjab.

WHO CAN APPLY
  1. For admission against Open Merit seats, only those candidates having an aggregate score of 77% or above marks can apply.
  2. According to PMDC Regulations and in compliance with the directives of Apex Courts, aggregate marks shall be calculated by adding the marks as follows:
    Matric or Equivalent ------ 10%
    FSc or Equivalent ------ 40%
    UHS Entrance Test ------ 50%
    The formula for calculation of Aggregate Marks is as follows:
  3. The aforementioned condition is not applicable to candidates seeking admission against any category of reserved seats (Disabled Students, Under-Developed Districts, Cholistan) who can apply if they fulfill basic eligibility criteria as laid down in the Prospectus.
  4. Candidates claiming to be Muslim Hafiz-e-Quran shall have to pass a test conducted by a Committee, constituted by the Admission Board, to attract the benefit of 20 marks which will be added to their FSc(or Equivalent) score. Only those candidates, who will secure at-least 75% aggregate marks according to the above formula, will be invited for the said test. All such candidates are advised to contact Prof I.A.Naveed, Director Medical Education, Room No.44 at UHS Lahore, in person, on or before 26th September, 2011, so that they could be registered for the Hifz-e-Quran Test.
HOW TO APPLY
  1. The Admission Kits containing Prospectus and Admission Form, will be available, free of cost, at University of Health Sciences, Khayaban-e-Jamia Punjab, Lahore, from 05th October to 08th October, 2011, from 09:00 am to 03:00 pm. The candidates will have to provide the attested copies of their Matriculation, FSc (or Equivalent) and Entrance Test Result Cards to be able to get the Admission Kit.
  2. Candidates must submit the duly filled Admission Forms alongwith all required documents as provided in the Prospectus, by hand at University of Health Sciences Lahore from 10th October to 15th October, 2011, from 09:00 am to 03:00 pm. Admission forms received by post shall not be entertained.
  3. Candidates will give their preference for medical and dental colleges in one single admission form which once given shall be final and cannot be changed subsequently. One candidate can only submit one form. There is no need to submit separate forms for MBBS and BDS.
  4. Forms once submitted shall not be returned. Incomplete or unsigned admission forms will be rejected.

IMPORTANT

  1. No admission form will be accepted after 03:00 pm on Saturday, October 15, 2011.
  2. Once the admission form is submitted, no other document (e.g., revised marks certificate, domicile certificate, etc) shall be acceptable. The documents submitted with the form shall be considered as final.
  3. Candidates already admitted in any medical and dental institution and seeking fresh admission shall have to disclose their previous admission as per the procedure laid down in the Prospectus.
  4. Any candidate found to have domicile of more than one place shall be disqualified.
  5. Any candidate found to have submitted forged certificates or fake documents with the admission form shall be permanently debarred from admission to any medical/dental college.

ENTRANCE TEST-2011
For Admission to Public and Private Medical / Dental Institutions of the Punjab

The University of Health Sciences (UHS) Lahore, shall conduct the Entrance Test for admission in First Year MBBS / BDS in Public and Private Medical and Dental Institutions of the Punjab on Tuesday, 13th September, 2011 at 09:00 A.M.

As per Regulations of Pakistan Medical & Dental Council (PMDC), it is mandatory to appear in the Entrance Test of the Punjab for admission to any medical / dental college of the province.

  1. ELIGIBILITY CRITERIA FOR ENTRANCE TEST
    1. The candidate has passed Intermediate Science (HSSC/F.Sc) (Pre-Medical Group) or Equivalent Examination, securing at-least 60% (660/1100), unadjusted marks (excluding the marks of Hifz-e-Quran).
    2. Those awaiting result must provide a Hope Certificate (in original), issued by the Head/Principal of their concerned institution, clearly stating that the said candidate is expected to secure at-least 60% marks in HSSC or Equivalent Examination.
  2. WHO CAN APPLY
    1. The candidates having domicile of any district of the Punjab and Federal Capital Area (Islamabad), Azad Jammu & Kashmir and Province of Gilgit-Baltistan (Northern Area).
    2. Foreign students or dual nationals of Pakistani origin must take the Entrance Test (If they have not passed SAT II examination with a minimum score of 550 marks in each science subject i.e., Biology, Chemistry and Physics / Mathematics) for admission against seats reserved for them under Pakistan Technical Assistance Programme (PTAP) and Self- Finance Scheme (SFS).
  3. FORMAT OF ENTRANCE TEST
    1. The test shall consist of one paper containing 220 multiple-choice questions (MCQs)- four options with single correct answer- divided into Four sections, namely:
      1. Physics (44 MCQs)
      2. Chemistry (58 MCQs)
      3. English (General for language proficiency) (30 MCQs)
      4. Biology (88 MCQs)
    2. The duration of Test shall be 150 minutes (2½ hours).
    3. Each question carries 05 marks. Total marks, therefore, are 1100.
  4. SYLLABUS OF ENTRANCE TEST
    1. The syllabus of Entrance Test 2011 has been available on UHS website www.uhs.edu.pk. The candidates are advised to prepare for the Entrance Test according to this syllabus from their respective textbooks.
  5. CENTRES FOR ENTRANCE TEST
    1. Entrance Test 2011 will be held simultaneously in following cities:

      Bahawalpur , Multan, Rahim Yar Khan, and D.G. Khan. Lahore, Faisalabad, Sahiwal, Gujrat, Gujranwala, and Sargodha. Rawalpindi and Hassan Abdal
    3. Candidates shall mention on their Application Forms and Admittance Cards the centre city from where they wish to appear in the test.
    4. Centres will be allotted by UHS subject to administrative / logistic arrangements which shall not be changed subsequently.
  6. HOW TO APPLY FOR ENTRANCE TEST
    1. Entrance Test Application Kit will be available at UHS and public sector medical / dental colleges at Lahore, Multan, Bahawalpur, Faisalabad, Rawalpindi and Rahim Yar Khan, from 20th August to 7th September, 2011(on working days only) from 9.00 A.M. to 3.00 P.M.
    2. Dully filled Application Forms can be submitted, in the following way, from 20th August to 7th September, 2011(on working days) from 9.00 A.M. to 3.00 P.M.

      1. Attested copy of Matriculation/SSC or equivalent examination.
      2. Attested copy of HSSC/FSc (Pre-medical) or equivalent examination (Candidate having foreign qualifications, such as A-Level etc., shall provide the Equivalence Certificate issued by IBCC.)or Hope Certificate ( in original) from the Head/Principal of the institute.
      3. Attested copy of CNIC or B-Form issued by NADRA.
      4. Attested copy of Birth Certificate issued by the Union Council, if date of birth is not mentioned on Matriculation certificate
      5. Duly filled Admittance Cards (both copies). The cards must not be separated. Please paste two coloured ID size photographs on the two cards.
      6. Two self-addressed A-4 size envelopes with Rs 40/- postage stamps each.
      7. Three recent passport size, coloured photographs of the candidate. One to be pasted on the application form and attested on the front and other two, attested on the back, to be attached with the form.
      8. Attested copy of domicile certificate of the candidate. No other document shall be acceptable in lieu of domicile certificate. Foreign candidates shall provide the attested copy of their foreign passport.
      9. Huffaz-e-Quran shall present a certificate of Hifz-e-Quran, issued by a registered Madrassa.
    4. The Admittance Cards shall be issued on the spot.
    5. The candidates are required to submit Application forms by hand. Applications received through post shall NOT be entertained.

IMPORTANT

  1. No application for Entrance Test shall be received after 03:00 PM on 7th September, 2011.
  2. No candidate shall be allowed to enter the examination centre without the Admittance Card issued by the University.
  3. Any candidate found to have domicile of more than one place shall be disqualified.
  4. Any candidate found to have submitted forged certificates or fake documents at any stage of admission shall be permanently debarred from admission to any medical / dental college.
  5. Entrance Test for Punjab is also mandatory for admission to private sector medical / dental colleges of the Punjab.
  6. There is no minimum or maximum age limit for admission to any medical / dental institution.
  7. Candidates having foreign qualifications must provide the Equivalence Certificate issued by IBCC only.
The Government of Punjab has decided to conduct Entrance Test free of charge. No costs will be paid by the candidate at any level.

APTITUDE FEEDBACK FOR ENTRANCE TEST 2011

A compulsory feedback shall be administered to all candidates after the completion of Entrance Test 2011, collection and secure packing of the Question Papers and Response Forms. The feedback is for University and Government use only and SHALL NOT IN ANY WAY affect the merit of the candidates.

The process of Admission in Government Medical and Dental Institutions of the Punjab for Session 2011-12, shall start in August 2011. Prospective applicants are advised to ensure the availability of following documents:

  1. Domicile Certificate with recent photograph.
  2. Computerized National Identity Card or Computerized B-Form issued by NADRA.
  3. Certificates of all the examinations passed on the basis of which admission is sought (such as Secondary School Certificate, Higher Secondary School Certificate or Equivalent).
  4. Detailed Marks Certificates of SSC and HSSC (Pre-Medical Group).
  5. Eight (08) recent passport size coloured photographs.
  6. Applicant having equivalent foreign qualifications (O and A-Level, 12th Grade, etc.) must obtain Equivalence Certificate from IBCC.
  7. Those claiming to be Muslim Hafiz-e-Quran, shall have to produce a Certificate in this regard from a registered Madrassa.
The above documents shall be required at the time of submission of application form for the Entrance Test.

IMPORTANT

  1. The syllabus of Entrance Test 2011 has been available on UHS website www.uhs.edu.pk since September 2010. The candidates are advised to prepare for the Entrance Test according to this syllabus.
  2. The candidates should prepare for the Entrance Test from their respective textbooks, that is, F.Sc. candidates need not study Non-F.Sc. textbooks and Non-F.Sc. candidates need not study F.Sc. textbooks.
  3. The candidates are advised to refrain from taking preparatory courses offered by commercial academies.
