Desirous firms should submit their applications along with these documents i.e. General profile and date of establishment of firm and other details in the form of a company’s brochure, Details of relevant permanent Technical Staff along with their C.V’s. , Registration with Pakistan Council of Architects and with Pakistan Engineering Council, List of relevant jobs completed with their cost and jobs in hand, Certificate if the firm has not ever been black listed by any client, Performance Certificates of the relevant jobs completed, List of Litigation/ Arbitration, if any. , Financial Status, General Sale Tax number and its latest renewal, Rate Fee for these services with break-down for each stage may also be quoted separately
SUBMISSION OF COST OF TENDER DOCUMENTS
The interesting firms should have to pay Rs. 2000/- (Bank draft / pay order) from a schedule bank in the name of V.C. / C.E. – University of Health Sciences Lahore to obtain the tender documents for each set.
ISSUANCE OF TENDER DOCUMENT
Tender documents will be issued from Finance Department to authorized persons, holding Authority Letter, National Identity Card and deposit slip from the bank, from 05-02-2010 to 15-02-2010 during office hours i.e. from 8.00 a.m. to 4.00 p.m. No tender documents will be issued after 20-02-2010.
