University of Health Sciences Lahore invites applications for prequalification/ registration from firms/companies for the following categories:-

  1. The application form can be had from the office of the Director (Admin & Coord) during the working hours. The same can be downloaded from the UHS Website

  2. The participating firms/companies must attach bank draft/pay order for a sum of Rs. 1,000/- drawn in the name of University of Health Sciences Lahore as registration fee. The same amount can be deposited in UHS A/c No. 1711-6 at National Bank of Pakistan, Sheikh Zayed Hospital Branch, Lahore.

  3. The firms/companies already registered with UHS can apply for renewal of registration after paying Rs. 250/- as renewal fee.

  4. The firms/companies must be well-reputed and of good standing, registered with Sales Tax, Income Tax and should have sound financial position.

  5. For serial No. IV (lab items/chemicals/glassware and plastic ware) preference will be given to the direct importer/sole agents.

  6. For serial No. V (Furniture and Fixture) the firms must be well established and reputed manufacturers. Those having their own integral manufacturing base and seasoning plant / facility for seasoning wood would be preferred.

  7. The last date for the receipt of applications for registration with the University is 26-07-2008.

Prequalification Notice for Contractors
The undersigned invites reputable construction firms to apply for prequalification, submit with their credentials and documents (profile in bound shape) for the project given below

PROJECT

Construction of Hostel Facilities for Visiting Faculty at University of Health Sciences Lahore (Four storey plus basement)


T.O.R. OF THE APPLICATION

  1. Full name address, telephone / fax number and e-mail address of the firm.

  2. Organizational structure (Company or Partnership). Attach a certificate and particulars of the partners and Directors along with National Identity Card, certified copies of the articles / memorandum of association of partnership.

  3. List of equipment owned by the firm with ownership certificate. The works committee if required will visit site / stores for verification of the T & P.

  4. List and cost of projects in hand indicating the name address and telephone numbers of the client, consultant and scope of work. Detail of works with a minimum cost of Rs. 30.0 millions last completed along with completion certificate (verifiable).

  5. Up-to-date registration with Pakistan Engineering Council.

  6. Bio-data of key personals and technical full time staff.

  7. Financial Status and Bank References (verifiable).

  8. The firm who does not own steel shuttering, scaffolding pipes need not apply.

  9. National Tax Number and Income Tax clearance certificate for the last year.

  10. Affidavit that applicants firm is not black listed or disqualified by any Government/ Semi-Government department.

  11. Affidavit to the effect that firm is not involved in any litigation with any department in the past and present or if any case of involvement give details of litigation.

  12. Those contractors who have already been pre-qualified for other university works must also apply.

  13. Affidavit on stamp paper properly attested to be submitted by the contractor / firm that the statement referred above is correct and nothing has been concealed intentionally, this aspect will be verified by the university authorities. The university reserves the right to accept or reject any or all the applications without assigning any reason.

Having applied for pre-qualifications means acceptance of terms and conditions mentioned above. The above information supported by documents must reach the office of the undersigned on or before 21-01-2008.

Short Tender Notice - Electrical Work
Sealed tenders are invited from the approved, income tax registered and listed contractors with various Govt. departments and autonomous bodies for the work recorded below on item rate basis with the conditions given below:-

  1. Tenders shall be received and opened on 15th December 2007 at 10.00 hrs. in the presence of contractors or their authorized representatives who may attend the office of the undersigned.

  2. The deposit at call issued by an scheduled bank of Pakistan in the name of Chief Executive / U.H.S. for the amount 2 % of the total bidded amount should accompany without which it will not be accepted.

  3. The details and specifications for these works can be seen in the office of the undersigned during working hours on any working day.

  4. The works will be executed under the supervision of the Engineer incharge, strictly as per specifications issued by the Govt. of Punjab.

  5. As per Government present policy, the rates should be quoted in figures and words against each item of work contained in the BoQ attached to tender documents. The contractor should place his signatures with full name, address and category.

  6. A set of bidding documents (Tenders) for each work may be purchased from Finance Section (UHS) for a non-refundable amount of Rs. 1,000.0 (Rs. One Thousands Only) in each case upto one day before the date of opening the tenders.

  7. The contractor whose tenders are accepted is bound to start the work within five days from the accepted date, otherwise his deposit at call will be forfeited.

  8. The competent authority can accept or reject any or whole tenders without assigning any reason.

  9. The competent authority may increase / decrease the quantum of work at any time without assigning any reason and the contractors are bound to accept it.

  10. The confidential, incomplete, conditional, scored, over written and sent by mail tenders are liable to be rejected.
